Research Abstract

Laparoscopic orchidopexy is the procedure of choice for the diagnosis and treatment of the non-palpable testis in boys. Single incision laparoscopic surgery (SILS) uses a single incision at the entry point instead of the 3-5 traditional laparoscopic incisions. Twenty-one boys with median age 33 (range 9-256) months having 26 intra-abdominal testes (bilateral in 5 boys, right in 9 and left in 7) underwent staged Fowler-Stephens orchidopexy between August 2016 and September 2017 through a single infra-umbilical incision using flexible instruments. Mean operative time for the first and the second stage was 38.2 (range 20-75) minutes and 66.9 (range 30-120) minutes respectively. Intra-operative complications were in the form of surgical subcutaneous emphysema in one with no consequences and delayed ureteral thermal injury in another boy that required open surgical repair using a Boari flap. Mean hospital stay was 1.2 days. Patients were followed up from 3-16 (mean 11.1) months. One boy has a hypoplastic testis in the scrotum and testicular retraction to the neck of the scrotum in another boy. SILS orchidopexy provides the benefits of better cosmesis, less pain, short recovery with a high success rate.
