Do we need a small catheter drainage with chest tube after uniportal video-assisted thoracoscopic surgery for better drainage?

Research Abstract
Materials and Methods: It represents a pilot prospective study randomized trial from August 2019 to August 2020 who had undergone uniportal VATS procedures were divided into two groups as group A (chest tube with small catheter) and group B (chest tube only).
Results: Fifty-three patients underwent uniportal VATS only 30 where eligible to the study divided into two equal groups (18 males, 12 females). The mean age of patients in the small catheter with chest tube was 42.07±12.85 years, compared to 34.93±10.73 years in the chest tube only. Six different types of operation was done. Postoperative pain and hospital stays was nearly equal in both groups (p> 0.05). Postoperative residual effusion in immediate chest x-ray in small catheter with chest tube was managed by aspiration from small catheter and show improvement in follow up chest x-ray next morning (p< 0.05) but in chest tube only was management conservatively didn’t show improvement in follow up chest x-ray next morning (p> 0.05).
Conclusions: Small catheter with chest tube didn’t add more pain. Although more than half of the cases get aspiration from small catheter and show improvement in follow up chest x-ray next morning, this does not statically affect hospital stay.

Comparative study of aortic root replacement versus aortic root enlargement in small aortic root

Research Abstract
For patients with aortic stenosis (AS) and small annulus, aortic root replacement (RR) and aortic root enlargement (RE) are proven methods to prevent prosthesis-patient mismatch (PPM). Our goal was to compare the results of those two categories of operations. This prospective study used our hospitals’ database of aortic valve replacements from 2021 to 2024. Patients who had aortic stenosis treated with RE or RR were compared. The postoperative results and reoperations were compared. Kaplan–Meier survival estimation and log-rank test were used to compare time-to-event data.
Results
A total of 72 patients had aortic stenosis treated with either RE or RR. Thirty-eight (52%) and 34 (47%) patients underwent RR and RE, respectively. Effect of ostarine on glucose level, lipid profile, and osteoporosis in streptozotocin induced diabetic male rats

Research Abstract

This study aimed to evaluate ostarine effects on blood glucose, dyslipidemia, and osteoporosis in diabetic rats. Forty-eight adult male rats were divided into six groups (Control, Ostarine: 0.4 mg/kg daily orally, diabetes mellitus (DM), DO: diabetic rats received ostarine, DI: diabetic rats received insulin, DOI: diabetic rats received ostarine and insulin for 8 weeks). Microendoscopic laminotomy versus open interlaminar decompression for degenerative single-level lumbar spinal stenosis: A non-randomized prospective cohort study with short-term outcomes.

Research Abstract

Purpose:
Conventional decompression is an effective treatment for lumbar spinal stenosis (LSS) but is associated with greater tissue disruption and perioperative morbidity. Microendoscopic laminotomy (MEL) is intended to reduce surgical morbidity; however, concerns persist regarding operative time and complications such as incidental durotomy. This study compares both techniques in terms of clinical outcomes, operative parameters, and complication rates.

Methods:
This is a non-randomized prospective cohort study, conducted over three years (two years for patient enrollment and one year for follow-up). A total of 68 patients with degenerative single-level LSS were included; 34 patients underwent conventional interlaminar decompression (CILD), and 34 patients underwent MEL. Clinical outcomes were assessed using the visual analog scale (VAS) for back and leg pain and the Oswestry Disability Index (ODI) preoperatively and at multiple follow-up points. Operative time, intraoperative blood loss, and hospital length of stay were also recorded. Complication rates, including incidental durotomy and reoperation, were evaluated.

Results:
The MEL group demonstrated significantly lower intraoperative blood loss and shorter hospital stays compared with the CILD group, while operative time was significantly longer (p < 0.001). The MEL group showed significantly better VAS back pain scores at the immediate, 3-month, and 6-month follow-ups; however, no significant difference was observed at one year. Similarly, ODI scores were significantly improved in the MEL group at the immediate and 3-month follow-ups, with no significant differences at later follow-up points. There were no significant differences between the groups regarding the incidence of dural tear or reoperation rates.

Conclusion:

MEL provides superior early postoperative recovery but requires technical expertise and is associated with a learning curve that may increase the risk of incidental durotomy during the initial experience. Both groups have similar late follow-up results.
