Effect of high-quality nursing care on postoperative complications and quality of life for patients undergoing common bile duct exploration

Research Abstract

Background Complications following common bile duct exploration for managing gallstones or choledocholithiasis
negatively impact patients’ quality of life. Occasionally, high-quality nursing care is necessary to either improve the
outcome or to avoid life-threatening consequences.
Aim This study aimed to evaluate the effect of high-quality nursing care on postoperative complications and quality
of life for patients who underwent common bile duct exploration.
Patients and Method A quasi-experimental research design was utilized. The study was conducted in the
Hepatobiliary Surgical unit at Al-Rajhi Liver Hospital and the general surgery department at Assiut University Hospital.
A purposive sample of sixty adult patients, whose ages ranged from 20 to 65 years, who underwent common
bile duct exploration were included in the study. Patients were randomly divided into two equal groups (study
and control) 30 patients for each. Tools: Tool (I): patient’s assessment form, Tool (II): Postoperative complications
evaluation record, and Tool (III): Abdominal surgery impact scale.
Results Wound infection and T –Tube problems demonstrated a statistically significant difference between the two
groups on follow-up as it occurred in (36.7%, and 26.7%) of the control group compared to (6.7%, and 3.3%) of the
study group. Also, there was a significant improvement in total mean scores of QoL among the study group as it
increased from 54.4 ± 22.11 on pre-intervention to 77.8 ± 6.15 post (P.value 0.001**).
Conclusion High-quality nursing care proved to be effective in reducing the incidence of postoperative
complications and improving quality of life among the study group compared to the control group.
Recommendations: Nevertheless, high-quality nursing care is crucial and should serve as the basis for routine
nursing care for patients undergoing common bile duct exploration.

Short-term outcome following postoperative enhanced recovery implementation in patients with perforated peptic ulcer

Research Abstract

Objectives

To evaluate the short-term outcome following postoperative enhanced recovery implementation in patients with perforated peptic ulcer.

Methods

Quasi-experimental research design was utilized. Thirty patients received postoperative enhanced recovery after open surgical repair of perforated peptic ulcer compared with control group who received routine care. Patient assessment sheet and gastrointestinal quality of life index were the tools used for data collection.

Results

The mean age was 40.43 ± 8.39 years for the study group and 39.53 ± 8.08 for the control group (56.7%, 70%), respectively, were males. The study group demonstrated early first bowel movement, flatus and stool passage (8.1 ± 1.16 (h), 12.6 ± 2.46 (h), and 2.47 ± 0.82 (days)), respectively, compared to control group (10 ± 1.11, 15.1 ± 2.04, and 3.57 ± 0.82). A significant reduction (6.93 ± 1.29 vs. 12.3 ± 4.96 (days)) and (30% vs. 60%) in hospital length of stay and postoperative complications among study group compared to control group (P < 0.01). The mean scores (56.17 ± 13.78 and 72.6 ± 11.89 vs. 34.33 ± 8.91and 53.43 ± 16.14) of gastrointestinal quality of life index were significantly better in study group (P < 0.05).

Conclusion

Improved gastrointestinal functional recovery, reduced postoperative complications, and improved quality of life, all were a result of implementing postoperative enhanced recovery among patients with perforated peptic ulcer.

Telehealth and challenges of statin adherence in patients with diabetes: a randomized controlled trial

Background
The success of any therapeutic regimen can be achieved only if patients adhere to the prescribed regimen. Finding an effective intervention to improve patient adherence is challenging.
Aim
To evaluate the effect of telehealth through short messaging services (SMSs) on adherence to statins in patients with diabetes.
Methods
This was a randomized clinical trial. Patients with diabetes mellitus were referred to the Center of Diabetes and Endocrinology at Assiut University Hospital and were assessed for eligibility. Ninety participants were randomly assigned to either the intervention or control group. Reminder SMS messages were regularly sent to the intervention group. Follow-up was performed at 6 and 12 weeks. Research Abstract

Background: Neurogenic dysphagia is one of the most common dangerous disorders for patients with neurological diseases. Aim: The aim of the study was to evaluate the effect of swallowing exercises on swallowing function for patients with neurogenic dysphagia. Patients and Methods: Research design: Quasi experimental (pre/post test) research design was used. Sample and Setting: A selected sample of 60 patients diagnosed with neurogenic dysphagia was recruited and also followed up for 2 months at neurological department and neurological outpatients clinic at neurological, psychiatric and neurosurgery Assiut University Hospital. Each patient was oriented and trained by the researchers to know the effect of practicing swallowing exercises and received brochure to be oriented about its effect on swallowing function. Tools: Patients assessment questionnaire, functional oral intake scale, Bazaz dysphagia score, eating assessment tool-10 and the 8-point penetration-aspiration scale. Results: The study finding revealed a highly statistical significant improvement (p < 0.0001) post application of the swallowing exercises as regard all outcome measures; functional oral intake scale, Bazaz dysphagia scale, aspiration and complications. Conclusion: Swallowing exercises significantly improve swallowing function and decrease severity of neurogenic dysphagia, aspiration and complications. Recommendation: The swallowing exercises (brochure) should be utilized in neurology hospitals as a teaching guide for patients with neurogenic dysphagia.

Effect of a Specific Exercise Program Post Temporomandibular Joint Arthrocentesis on Symptoms Severity, Pain, and Mouth Opening

Background
 
Post arthrocentesis exercise program has a positive effect on symptoms, pain and the range of joint motion.
Aim
 
to evaluate the effect of a specific exercise program post temporomandibular joint arthrocentesis on symptoms severity, pain, and mouth opening.
Research design
 
A quasi-experimental research design (study/ control groups) was utilized.
Subjects
 
A sample of 40 patients.
Setting
 
The study was conducted in the Oral and Maxillofacial Surgery Unit and outpatient clinic at Assiut University Hospital.
Tools
 
Four tools were utilized: Tool I: Patient's assessment sheet included demographic data; Tool II:  TMD questionnaire; Tool III: Wong-Baker pain rating visual analogue scale; and Tool IV: Range of maximal mouth opening measured in mm.
Results
 
There was a statistically significant difference between the study and control groups regarding temporomandibular disorder mean score at 1 month (5.65±3.48&9.75±4.99) and 3 months (2.05±4.3&7.05±4.24) postoperatively with p.value (0.005**& 0.001**) respectively. A significant decrease in the mean pain score for both groups at 1 month and 3 months compared with the preoperative period and 1 week postoperatively with  p.value (0.004**& 0.0001***). Also, there was a high statistically significant difference between the study and control groups regarding the mean score of the range of maximal mouth opening measured in mm (37.7±3.66& 33±4.58) at 3 months postoperatively with p.value (0.001**).
Conclusion
 
The exercise program post arthrocentesis is more effective in decreasing symptoms severity, and pain and improving mouth opening.
